GPH does not collect personally identifiable information about individuals except when such individuals specifically provide such information on a voluntary basis. For example, such personally identifiable information may be obtained when a request is submitted in the Register for Information section. In the Contact portion of this Web Site, you can make requests for more information by providing us with your contact information. No personal information should be provided in this Web Site by minors under the age of 18.
